The tipping concept, action, habit, in the States is endemic and it must be abolished as is. If offering an extra remuneration—either small, or big—for services well rendered is to have a true meaning, one most not do it as an obligation, nor should it ever be added to a tab as a default.
Employees must get paid without having to count on tips as a source of income. It is not. It is a reward.

Tipping as a universal concept has evolved to attempt to solve local, systemic problems, as you’ve hinted at, and no longer resembles that sentiment you desire in any of the countries I’ve ever visited; I’ve had to do that as an extra, on top of local custom, without exception, to the best of my recollection. A daunting task, as @prologic@twtxt.net says!
